Yes. USCIS rejects I-485 applications filed without a signed, sealed I-693. Complete your exam first, then file.
Two. The first is a quick blood draw. The second, about 3–5 business days later, is your physical exam, any vaccines, and your sealed form.

A positive blood test doesn't mean you have active TB. You'll need a chest X-ray. If it shows anything that needs follow-up, the civil surgeon refers you for evaluation before the I-693 can be completed. We guide you through each step.
No. Eat normally and drink plenty of water.
No. We use the Quantiferon TB Gold blood test, which isn't affected by a past BCG vaccine the way the older skin test can be.
Come anyway. The doctor reviews what you have and gives only the age-appropriate vaccines you still need. In some cases a blood test (titer) can show you're already immune.
For forms signed on or after November 1, 2023, the I-693 stays valid as long as the I-485 it was filed with is pending. If that I-485 is withdrawn or denied, you'll need a new exam for a new filing.
The Quantiferon TB Gold, RPR and gonorrhea tests, the physical exam by the civil surgeon, and your signed, sealed Form I-693. Vaccines are extra and only given if your records show you need them.
The immigration exam itself isn't covered by health insurance.
